On December 8, 2025, Immigration, Refugees and Citizenship Canada (IRCC) unveiled a historic three-pronged approach to tackle Canada’s severe shortage of medical professionals. These new measures create the clearest, most predictable, and fastest permanent residence (PR) pathway ever offered to internationally trained doctors.

The core goal is simple: to retain doctors already working in Canada and give provinces extra capacity to recruit practice-ready physicians without affecting their existing quotas.

Here is a detailed breakdown of the three key measures, which take effect in early 2026.

1. New Dedicated Express Entry Category: Physicians with Canadian Work Experience

IRCC has introduced a new, permanent category under Express Entry specifically targeting foreign-trained physicians who are already licensed and working in Canada on temporary status.

This move creates a new “in-Canada” pathway that resembles the successful Canadian Experience Class (CEC) model.

Eligibility Requirements:

  • Work Experience: Applicants must have at least 12 months (1 year) of full-time, continuous work experience (or an equivalent amount of part-time experience) in Canada.
  • Timeframe: This experience must have been gained within the last three years.
  • Eligible Occupations: The category is restricted to the three primary physician roles:
    • General practitioners and family physicians (NOC 31102)
    • Specialists in surgery (NOC 31101)
    • Specialists in clinical and laboratory medicine (NOC 31100)

Why this is a Game Changer:

This new category will run alongside the existing, broader Healthcare and Social Services category. The crucial difference is that the new “Physicians with Canadian work experience” category is highly targeted and requires a longer period of proven, in-Canada practice. This means doctors who are currently practicing but may have lower CRS scores (perhaps due to age or single-language proficiency) now have a much more secure pathway to PR than relying on a general Express Entry draw.


2. 5,000 Reserved PR Admission Spaces for Provinces 🩺

To empower provinces and territories (who manage healthcare licensing) to recruit doctors directly, the federal government has created a new, non-competitive allocation of PR spaces.

  • The Allocation: 5,000 federal admission spaces will be set aside annually for provinces to nominate licensed doctors who have job offers.
  • The Crucial Detail: These 5,000 spots are in addition to the regular annual allocations provinces receive under the Provincial Nominee Program (PNP).

Why this is a Game Changer:

Traditionally, when a province nominates a candidate (including a doctor), that nomination reduces its overall annual PNP quota. By ring-fencing these 5,000 spots, provinces can drastically increase their intake of doctors without having to cut nominations for other critical workers like nurses, truck drivers, or tech professionals. This allows provinces to act swiftly on licensed, practice-ready physicians who have committed to local practice.


3. Expedited 14-Day Work Permit Processing 📅

For foreign doctors who receive a Provincial Nomination (whether through the existing PNP or the new 5,000 reserved spaces), IRCC will expedite their work permit application processing to just 14 days.

  • The New Standard: Nominated practice-ready physicians can receive a new work permit or an extension in approximately two weeks.
  • The Impact: This drastic streamlining cuts processing times that can typically take several months. It allows doctors to begin or continue treating patients immediately while their full Permanent Residence application is being finalized—a process that can take up to 8 months under Express Entry.

Together, these three measures establish a comprehensive, end-to-end strategy for doctors:

  1. Work Permit: Nominated physicians get fast-tracked work permits (14 days).
  2. Permanent Residence: They can apply for PR via a dedicated Express Entry category OR through an additional PNP space (which adds 600 CRS points).
  3. Retention: The new Express Entry category incentivizes those already working in the country to stay and apply for PR.

This announcement is a monumental step toward filling Canada’s urgent healthcare gaps by providing qualified international physicians with the stability and certainty they need to commit to Canadian practice long-term.
